All students and adults must have a completed SummerSalt Medical Release Form before arrival. For South Carolina churches, these forms only require a parent’s signature for students under 18. For churches outside of South Carolina, these forms require a notarized parent’s signature for students under 18. Please check to make sure you have enough forms. We will need two copies for each student/chaperone at check-in. We are happy to mail additional forms as needed. Students will not be able to participate in any events until a properly completed form has been secured. When arriving for check-in please group these forms in alphabetical order and separate them by color to expedite the registration process.
