Hurricane Harvey

  • The Pine Level Baptist Church (Savannah River Assn) recovery unit is scheduled to serve in Orange, TX, Jan. 21-29. Unit leader is Gary Mixson, with team members joining him from across the state. They will do mud-out and rebuild.

Why We Serve

Baptist Disaster Relief strives to bring “help, hope, and healing” to people in crisis. Physical help may be in the form of cutting trees, serving food, or spraying for mold. That gives us the opportunity to also give emotional and spiritual help, offering the hope that comes from a relationship with Christ.

As a Unit Leader, make sure that every person on your team is ready to share the hope of Christ when opportunities arise. For a guide to holding a one-hour training session for short-term missions volunteers (which is what your team members are!), please click HERE.

Responding to an Event

  • Units are encouraged to respond to local community needs whenever you are aware of them. Please inform the state DR office when your unit activates locally.
  • Events in which you would travel to another area of the state, or out-of-state, are coordinated through the state DR office. Please contact us if your unit is available to serve in a call-out; do not self-deploy outside of your local community.
  • Please PRAY about what God would have you do to share His love!


I.D. CARDS: If you wish to come to the Convention building to renew your card, please call ahead to make sure someone will be in the office to do it.

STANDARD INFO:  If your unit does not have a designated chaplain, please advise the Disaster Relief Office and one will be attached to your unit for each response.

South Carolina Disaster Relief Forms for Unit Leaders

Each South Carolina Disaster Relief (SCDR) Unit Leader should become familiar with the forms they are responsible for completing when being deployed to a disaster site. Forms and explanations on their use are included for your convenience. You may download a copy of the form to print by clicking on the “Download” button.

SCDR1: Personal Information Update– 2016

Have all volunteers participating on a deployment complete form SCDR1. The SCDR1 must be kept on your unit while you are deployed, in case of accident or injury to the person. You may be asked to make copies of these forms for the On-Site Incident Commander. After returning from deployment, send your team’s SCDR1 forms to the state DR office. (Note: this is a change from previous practice, due to liability issues.)

SCDR1a: Personal Information for Non-DR volunteers

If a “Spontaneous Untrained Volunteer” (SUV) works with your team, have them complete this form/waiver, andsend it to the state DR office along with your team’s Personal Info Updates after the deployment.

SCDR2: Roster of Volunteers 2016

The Unit Leader will complete and turn in a team roster. There are several ways to do this – choose one of these ways:

It is very important that this form is completed and turned in before you deploy because it is used to write up insurance for your team.

SCDR2a: Sign-In Sheet for Non-DR Volunteers

If untrained volunteers (SUVs) work with your team, have them sign in and out on this form and document their hours. We will not provide insurance for these folks, but we need to know how many volunteer hours went into the job, including SUV hours. Please send completed form to the state office by mail, fax, or email, along with the SCDR1 and 1a Personal Info Forms.

SCDR3: Property Owner Request for Assistance- 2016

Any request for help is recorded on SCDR3. This form then becomes a work order. This work order/assignment is taken to the Command Post, and the Incident Commander then gives the assignment to the appropriate Unit Leader . When the unit has completed the assignment, the SCDR3 is returned to the Command Post assignment desk and is filed. Keep updated blank copies of these forms on your unit in case someone walks up and asks for assistance. FYI: Protocol is that they call the Command Post with their request; so if you are asked to assist someone before you leave the area, call the Command Post to get permission from the Incident Commander before proceeding.

NOTE: Before beginning any assignment, written permission of the property owner MUST be obtained on form SCDR3.

SCDR3a: Tenant Request for Assistance

Use this form if the resident does not own the home. The homeowner must also fill out Form SCDR3, so you have permission from both the resident (renter) AND the owner.

SCDR4: Assessment Forms

Assessment teams should take and complete SCDR4 forms as they assess damage. These forms are used to record the job type and determine the extent of damage, size of the job, type of tools, equipment, and materials needed. These forms accompany the SCDR3,  and should be turned in to the Command Post so that adequate units and equipment may be secured.

SCDR5: Daily Report Form

Unit Leaders must submit daily reports of relevant information (food counts, types of assistance offerered, jobs completed, etc.). Daily Reports may be submitted in two ways:

  • via Formstack on your smartphone or computer at this link: https://SBDR.formstack.com/forms/daily_report
  • by printing out copies of Form SCDR5, taking them with you and filling one out each day by hand. You will then submit the report to the On-Site Incident Commander or their administrative assistant – have them make a copy for you to keep. If you have served out-of-state, when you arrive home, send a copy of all your daily reports to the Disaster Relief Office, SC Baptist Convention, 190 Stoneridge Drive, Columbia, SC 29210.

Accident/Incident Report Form

An Accident/Incident Report Form should be completed any time there is an accident/incident during a deployment. This is while en-route, on site, or returning. Make sure that this form is completed and sent to the Disaster Relief Office (SC Baptist Convention, 190 Stoneridge Drive, Columbia, SC 29210 / FAX: 803.799.1044 Attn: DR / sueharmon@scbaptist.org ) immediately following an accident.
